Insurance Information
Insurance policies play a key role in your journey for weight loss surgery. These policies will determine whether bariatric surgery is a covered benefit of your policy. They will also tell us what your specific guidelines are to qualify for the gastric sleeve such as supervised weight loss, special testing, and any other requirements necessary before the insurance company approves your surgery. In order to determine your policy coverage, you can call the number on the back of your insurance card and provide them with the following information: Gastric Sleeve Procedure: CPT 43775
