Notice of Privacy Practices

At St. Mary's Medical Center we are committed to using and disclosing protected health information about you responsibly. This Notice of Privacy Practices describes the personal information we collect, and how and when we use or disclose that information. It also describes your rights as they relate to your protected health information. This Notice is effective as of April 14, 2003, as revised effective June 17, 2013 and April 15, 2015, and applies to all protected health information as defined by federal regulations.

This notice shall apply to the following locations: St. Mary's Medical Center, and all outpatient programs and services offered under the name of St. Mary's Medical Center on the hospital campus and at various other locations in West Virginia, Ohio and Kentucky. Examples of such outpatient programs and services include the St. Mary's Family Medical Centers and Urgent Care Center as well as provider-based departments of the Hospital. This notice shall also apply to the St. Mary's Medical Center Home Health Agency.

Unless otherwise identified as such, members of the Medical and Dental Staff, non-physician providers, residents and students are not employees of St. Mary's Medical Center, and this Notice is not intended to imply that such a relationship exists.
