Qualifications
- Doctor of Pharmacy Degree from an ACPE-accredited school of pharmacy
 - Eligible for licensure in the state of West Virginia
 
Required Documents
- Three (3) standardized references completed on the PhORCAS Recommendation Form.
- It is highly encouraged for all three letters to be from a clinical preceptor, research mentor, or faculty member who has spent a significant amount of time with the candidate and is able to speak to their clinical abilities, strengths and overall performance.
 - Letters written by people not in the medical field are highly discouraged.
 
 - Letter of intent stating why the candidate desires a residency at St. Mary's Medical Center.
 - Official transcript.
 - Current Curriculum Vitae (must include completed and anticipated rotations).
